Lockout Tagout: What Most Plants Get Wrong

Almost nobody gets hurt running a machine. They get hurt cleaning it, servicing it, or clearing a jam. And the failures behind those injuries live on paper, not in the hardware.

Ask anyone who has spent a career on a plant floor, and you’ll hear the same thing: the danger almost never shows up while the machine is doing its job. It shows up the second a person reaches into it, to wipe it down, free a stuck part, or clear a jam. In that moment, energy that should have been locked out is often still live, and that is how people lose fingers, hands, and lives.
Lockout tagout (LOTO) is the discipline that stands between a routine task and a tragedy. It is the process of confirming that every source of hazardous energy, electrical, hydraulic, pneumatic, gravity, thermal, and chemical, has been isolated and released before anyone reaches into the machine. The rule is simple to state. Applying it correctly on every machine, on every shift, by every worker is where it quietly falls apart.
The State of Lockout Tagout in 2026
The numbers haven’t budged for a decade, and they’re moving in the wrong direction.

Lockout/tagout has ranked among OSHA’s six most-cited standards every year for a decade, and it is still the single most-cited regulation on the manufacturing floor. A hazard we have understood for fifty years should not sit at the top of that list. It does because the gap isn’t in the technology or the science. It is in the distance between what the standard requires and what actually happens on the floor, shift after shift.
Companies Fail on Paper, Not on Parts
Here is the part that catches most managers off guard. The three most-cited pieces of the lockout standard have nothing to do with locks, hasps, or padlocks. They are all about paperwork and proof: the written procedures, the training records, and the annual check that the program still works the way it is written. Companies buy the hardware. What trips them up is documenting and verifying that it gets used correctly.

Why does this keep happening? Rarely because anyone is cutting corners on purpose. Most companies fall short because they don’t know what they don’t know. The standard is far more demanding to apply than it looks from the outside, and the gaps stay invisible until an auditor writes them up, or an injury exposes them the hard way.
A written procedure that lists only broad steps, such as locking out the electrical, the suction, and the return, then bleeding it off, can feel complete, but OSHA wants the specific isolation points identified: which breaker, which valve, in what order. Training is another common shortfall. A proper authorized-employee class runs two to four hours; plenty of facilities cover lockout in a ten-minute internal session with no field verification at all. And periodic inspections, the annual requirement to actually watch someone perform the lockout, get treated as an administrative box to check, or skipped entirely.
A Real Example: Three Hours Into Her First Shift
Numbers make the scale clear. One story makes the stakes real. Consider what happened to Kina Repp.
Kina Repp was a college student in Washington State when she took a job at a fish-processing plant just north of Seattle. On her very first day, she was placed on a line feeding fish carcasses into a shredder. When the fish jammed, she reached in to clear it. She had been given no training on the equipment or the emergency stops, and no one had told her to keep her hands away. Three hours into her shift, the machine pulled her in, and she lost her arm just past the elbow.
Kina Repp survived, recovered, and went on to become a national advocate for lockout/tagout, turning the worst day of her life into a warning for everyone who works around hazardous energy. Her story is the clearest possible answer to why this work matters: no procedure, no training, no lock. It was preventable in every way that counts.
Why Jam-Clearing is the Moment That Kills
Kina’s story is not an outlier. It is the pattern. Read enough OSHA case files and the same fact repeats: almost no one is hurt while the machine is running. Running it keeps your hands clear of the point of operation, the place where the equipment actually meets the product. Cleaning it, servicing it, or clearing a jam puts them right into it.
Jam-clearing is especially deadly because it happens so often. The jams that hurt people tend to be the ones cleared the same way eight to twelve times per shift, a sign the equipment has an unsolved engineering problem. Full isolation can be slow or awkward because of how the disconnects were installed, so under production pressure, crews take the fast path: reach in, clear it, move on.
The trap: Nearly all of the time, nothing goes wrong, which is exactly why the shortcut becomes routine. But a decade of getting away with it doesn’t make the next reach-in any safer. The odds only have to fail once.
What Good Lockout Tagout Looks Like

The Cost of Getting it Wrong

And the penalty is only the beginning. After a serious incident, a plant faces shutdowns, cleanups, rewritten procedures, retraining, re-inspection, follow-up OSHA visits, repeat-citation exposure, and often litigation: a year or more of disruption that touches every employee, not just the one who was hurt. When a program is this weak, the question stops being if someone gets hurt and becomes when.
